Use this form to register loss or damage of a school-supplied or a BYOD device.

So that you are clear on the process and details, warranty (either 12 months standard or 3 years AppleCare) covers malfunction of the device from manufacturing defects. Warranty conditions do not cover accidental damage, the most common examples being dropped devices or liquid damage.

Process:

  1. Damage is reported via this form and device is returned to the ICT Department (Level 3, Foundation Building) for assessment.
  2. If the person handing in the device or filling in the form is a student, parents/guardians will be informed of the damage as acknowledgement of the situation.
  3. ICT staff will pass the device to one of our three repairer vendors for quotation on cost.
  4. On receipt of a quotation, ICT staff will inform the parents of the cost of repair. Parents then acknowledge receipt of this information towards payment.
